In order to deliver better care at lower costs, physicians will need to begin providing patient-centered care, making use of the latest technologies like electronic health records, experts say.

As the healthcare system becomes more complicated, it is also growing more fragmented. This means that a primary care physician may not necessarily be aware of how their patient's visit with a specialist goes, which can make the individual's next trip to the primary care doctor less efficient.

However, as more doctors begin using electronic health records to provide patient-centered care, these problems will begin to dissipate, said Seven Weinberger, the executive vice president and CEO of the American College of Physicians, according to Healthcare IT News.

"That means there must be coordination of care across providers, and it must be seamless so the ball is not dropped through the patient-centered medical home and the medical home neighborhood," he told the news source.

The Patient Centered Primary Care Collaborative states that patient-centered medical homes are among the most promising models of care for controlling rising healthcare costs. EHRs will play a central role in these types of systems.
